[vhdl-200x-ft] Revised FT16: context declarations and clauses

Folks,

Attached is a revised version of FT16, context declarations and context
clauses, addressing issues discussed in today's telecon:

- Revised to allow multiple context references and mixing with library/use
  clauses in context clause of a design unit.

- Simplification of grammar - context declaration just contains a context
  clause.

- Simplification of static semantics.

- Corrections/revisions to example context declarations.

- Discussion of analysis dependencies.
