[vhdl-200x-ft] Another update

Now we are up to revision 4.

Changes in this release:
rising_edge and falling_edge for bit (FT19)
tee function changed (FT5A)

fixed and floating point packages reformatted for easier
reading. I also did a synthesis test on these packages,
which I got up to 100 MHz in floating point and 140 in
fixed point.
