[vhdl-200x-ft] New code release

85,180 lines of VHDL code. I consolidated some things and removed
some redundant test. At this point I have finally finished off all
of the VHDL code action items I have taken, and now I'm going to do
the documentation part of it (yes, updating the proposals).

Notes:
Please look at the "standard_textio_additions_body.vhd" to see
how I implemented the to_string(real, format) and to_string(time,resolution)
Also, the code presented in FT13 (VHPI packages) won't compile, so
it is modified here.
Everything is now layered.
numeric_std depends on std_logic_1164
fixed_pkg depends on numeric_std
fphdl32_pkg depends on fphdl_base_pkg and fixed_pkg

"compile.mti" will compile and run everything in the MTI environment.
