[vhdl-200x-ft] New VHDL packages code release

I just updated:

Major changes from last time
1) Every file now has it's RCS revision number beside it (would a date
be better?)
2) I have written and updated several of the testbenches, and fixed
     a fair number of bugs in the main package.
3) The fixed point packages are now where the proposals want them
     to be. They are fully functional, and have been tested in synthesis.

We are up to 87,487 lines of code now, and still growing.
