[vhdl-200x] Action Item (All): Fill in TWIKI information

Hi,
For the working group roster, for each member we need
to provide IEEE with your affiliation and email address.

Since you are already on the reflector, I have the email
address, but it would be much easier if we kept this stuff
in your TWIKI account. You can include a no-SPAM in your
email to keep out spammers - maybe someone can suggest a
convention - such as upper case letters are not part of the
email address - please make it easy or if you need something
more complex, perhaps you can volunteer to maintain the
roster :)
