I have followed up with the folks and IEEE and got approval to put
P1076-2008/D4.3a on the Accellera VHDL-TC document repository. You can find
it at

http://www.accellera.org/apps/org/workgroup/vhdl/documents.php

This is for use by VHDL-TC members as part of the process of development of
the standard. It is not for general publication.
