[vhdl-200x] Please proof read corrected Annex A

Dear colleagues,

While editing the VHDL LRM, I found that the folks at IEEE-SA had managed to
mangle the contents of Annex A, Syntax Summary.  All of the productions that
spanned multiple lines had the "::=" symbol missing.  For those productions
that fitted on just one line, they'd dropped trailing non-letter characters
(eg, ";", "}", "]", etc).

These are the only kinds of manglings I found, and I've corrected them based
on my knowledge of the grammar.  However, I would very much appreciate a few
more eyes checking for things I've missed.  Please see
