[sv-ac] 1995 fixes for Shalom's comments

I have posted an updated 1995 proposal with these fixes.  Since they are
all phrasing issues, rather than really changes of substance, I'm hoping
we can get the changes approved by voice vote tomorrow.  I have also
attached the .doc file with change tracking.

don't do anything. I think you mean something like, "The assertion does
not use sampled values of the loop variables."

But otherwise, it looks ok.
