[sv-ac] SV-AC feedback on D4 implementation

Hi Stu:

SV-AC had a voice vote in our meeting today to approve the attached
feedback on the D4 implementation.

These are all minor changes.  Some are the result of minor flaws in
the proposals rather than in the implementation, so we voted to
ensure that all are approved by the committee.
