Hi Doron:

I considered adding a somewhat laborious statement in Clause 16
defining "the actual argument that corresponds to a formal argument"
as either the one from the instance that is bound by name or by 
position to the formal argument, or, if no such argument exists in
the instance, then the default actual argument from the declaration 
(which must exist in this case).

In the rewriting algorithm, we must understand the corresponding 
actual argument in this way.

If people think that this needs to be added more explicitly, it should
be easy to do it.
