[vhdl-200x-ft] Another code release (V12)

Just uploaded release 12.

Changes in this release:
User's guides for fixed and floating point (Fixed_ug.pdf and Float_ug.pdf)
Major updates to the fixed point packages.
Modified the floating point REM and MOD functions to use much less logic.
Fixed a bug in the 1164 "read" routines.
Testing on this release has been done with 3 simulation tools and 4 synthesis tools.

Next release:
I will be adding package generics to the fixed point package, and doing
some name changes:
fixed_pkg => fixed_base_pkg
fixed_generic_pkg  -  New, fixed_base_pkg w/ package generics
fphdl_pkg => fphdl_generic_pkg

132,998 lines of code and counting (2,990 more that last time).
