[vhdl-200x-ft] fixed point generics

I suggest using package generics to set the values of the five default 
behavior constants (fixed_round, fixed_truncate, fixed_saturate, 
fixed_wrap, and fixed_guard_bits) within the fixed point package 
fixed_pkg.  That option would be convenient for users who intend to use 
non-default values throughout a design.  Besides abbreviating calls to the 
long version of the divide function, the new types created by the package 
instance would help ensure that all uses of the fixed point operations are 
consistent within a design.
