[vhdl-200x-ft] Another code release (V11)

Just uploaded release 11.

Changes in this release:
I have created VHDL-93 compatibility packages where those are practical.
They are in:
http://www.eda-twiki.org/vhdl-200x/vhdl-200x-ft/packages/additions_11.tar.Z
with a compile script and a README.  Designed to be compiled into a
library named IEEE_PROPOSED.

I have updated the ?= operator, and restored the "match" function.

The "numeric_unsigned" package has been replaced with "numeric_std_unsigned"
because there is already a "numeric_unsigned" package in some tools.

The fixed point packages have changed, particularly "*", "/", "rem", "mod"
and reciprocal routines.  They are now much more efficient, and the bounds
on the values they return have been fixed.

There are now Synthesis test for the fixed and floating point packages.
