[vhdl-200x-ft] FT16 Context Unit updated and FT31 FILE_FLUSH added

Hi,
Peter has completed FT31 file_flush.
I have added an action item for all to review it.

Peter has completed his action items wrt ft16 context unit.
I have marked it done per the notes at the 2/18/05 meeting.
