[sv-champions] Explanation for the duplicate review of 3 Mantis items

Hi Champions,

You may have noticed that the list of Mantis items that I sent to the
Working group is shorter than the list that we reviewed in the last
Champions meeting. Below is a copy of the explanatory note that I added to
3 of 6 Mantis items that were approved in the June 5th conference call.

The proposal was unanimously approved by the Champions in the
June 5th, 2008 conference call.

Note: It was also approved by the Champions in the May 22nd conference call.
       I neglected to update mantis with the Champion's result from May 22nd.
       The Working Group approval wasn't recorded until June 9, which was after
       I put together the agenda for the Champions meeting on June 5th. The
       Champion's ended up reviewing this proposal twice. Thankfully they
       came to the same conclusion both times.

The following 3 Mantis items had this same problem:

    2237
    1742
    1503

I got behind in my Mantis updates, which caused these 3 to be reviewed
twice.
