[sv-ac] Mantis 1591 review (Draft6)

Hi all,

 

I reviewed Mantis 1591 and found the following issues:

 

* The sampled value function BNF should be typeset consistently in
16.9.4 and in 19.13, the typesetting of 19.13 (red bold terminals)
should be used. Also note an inconsistent spacing in $sampled and other
sampled value functions in 16.9.4.
