[sv-ac] formal_list_item

All:

I just voted yes on 928 because "list_of_formals" is not referenced
on the right side of any production in the grammar.

Once list_of_formals is removed, I believe that formal_list_item 
attains the same status--it is no longer referenced on the right
side of any production.

And if formal_list_item is removed, then actual_arg_expr attains
the same status--it is then no longer referenced on the right side
of any production.

As long as we are cleaning up, I think we should remove these 
superfluous items as well.

But we should also review to make sure that the tf_port_list
allows the passing of event_expression and $ to a sequence 
or property as well as the assignment of default values in the 
formal argument list of a sequence or property.
