[sv-ac] Documents for SV-AC errata meeting

I am attaching two PDF files that describe:

1) IEEE patent policy - since SV-AC is operating under IEEE P1800 we
      must adhere to this.

2) SV errata committee operating guidelines - Karen Pieper was kind
     enough to put these together.

We will go over both these documents at the upcoming conference call.
