[sv-ac] errata based on draft5

Attached are the change requests based on the review of Draft5 LRM. Please
review and provide your comments. I plan to submit these requests tomorrow.
I'll continue to work on a separate errata list to add examples for 1)
operator_assignment in local variable attachments and 2) expression_or_dist
assumption usage for disable iff and if-else construct
