[sv-ac] Draft4 review. My items

I reviewed my part of Draft4.

Annex B, C, P - no issues found.

Wrong fonts in 16.14.6. Filed a new Mantis 4008 (http://www.eda-stds.org/mantis/view.php?id=4008).

Mantis 3113 - no issues found in addition to the issue reported by Shalom.
Added notes or edited existing notes to add more issues to the following Mantis items:
Mantis 3213 - note 12949
Mantis 3033 - note 12906
Mantis 2093 - note 13017
